MADISON TOWNSHIP — The Madison Local School District has partnered with FinalForms, an online forms and data management service.
FinalForms allows parents and guardian to complete and sign academic and athletic forms for their students electronically to will save time.
Madison We is beginning this process of transitioning to FinalForms with KINDERGARTEN REGISTRATION for the 2021-2022 school year.
All current parents with students in the District will receive an email from FinalForms Wednesday, March 24, 2021 to confirm an account created by FinalForms. Please contact the child’s school if you need to add or update your email before March 24th to make sure you receive this important communication.
Once you confirm your account with FinalForms, you may begin registering your new Kindergartener for the 2021-2022 school year on April 6. Parents will have one account that will service all of their students in the district. Please do not create a duplicate account for your Kindergarten Student.
Parents who have current students in grades K-11, but are not registering a new Kindergarten Student should confirm their account with FinalForms in the March 24th e-mail, HOWEVER, the forms for your students will not be available/updated until May 2021. Please refrain from filling them out at this time.
If you do not currently have a student attending Madison, you will need to create a new account with FinalForms. A link to create this account will be posted April 6th at www.mlsd.net.
KINDERGARTEN REGISTRATION for the 2021-2022 school year will take place April 6-30, 2021. The child must be five (5) years old on or before Aug. 1, 2021 to enroll for the 2021-2022 school year. Please follow the instructions below to register your child:
1. Complete Online Registration
2. Schedule an appointment with the Elementary School in which you reside.

3. Bring the following documents: child’s birth certificate, record of immunization, custody papers (if applicable), parent/guardian driver’s license or ID card and proof of residency. Proof of residency needs to be either a lease agreement/house payment or a major utility bill.
Required immunizations for school attendance are:
Four doses of DPT (5 doses if 4 were given before 4th birthday).
Three or more doses of Polio with final dose after 4th birthday.
Three doses of Hepatitis B.
2 MMR’s.
2 Varivax (Chicken Pox) or history of having disease.
