GALION — The state-mandated deadline for students in kindergarten through 12th grade to receive immunization shots is Wednesday, Sept. 7.

Parents must have official documentation of the vaccination, have an appointment scheduled or sign an exemption form by this deadline.  Required vaccines vary by grade level.

The Galion City Health Department has been holding extra immunization clinics to help families meet the new requirement. The regularly scheduled clinics for September are full, but two additional clinic days have been scheduled for any remaining students in need of immunizations:

Wednesday, Sept. 14 – 9 to 11 a.m. and 1 to 3 p.m.

Wednesday, Sept. 21 – 9 to 11 a.m. and 1 to 3 p.m.

Please call the Galion City Health Department at 419-468-1075 and have insurance information ready in order to schedule an appointment or to have questions answered. It is also encouraged to go to the GCHD website, www.galionhealth.org, download the “Screening Form” and fill it out in advance to expedite the appointment.

Specific vaccine information statements and the Screening Form are also available in the Galion Middle and High School offices.

Parents or guardians must accompany their student for consent/insurance/payment information, and in the event of a reaction or emergency.

The Galion City Health Department is located at 113 Harding Way East.
