MANSFIELD — Third Street Family Health Services has announced a Community Health Quality Recognition Badge for Advancing Health Information Technology.

“About two years ago, we embarked on a journey to focus on achieving the Health Resources and Services Administration (HRSA) Community Health Quality Recognition badges,” the agency stated. “These are some of the highest honors a Federally Qualified Health Center like Third Street Family Health Services can earn.

“We are extremely proud to share that we were awarded the Badge for Advancing Health Information Technology. This accomplishment reflects our commitment to enhancing healthcare delivery through innovative practices.

“To earn this badge, we had to meet several important criteria. We adopted an Electronic Health Record System, enabling us to streamline patient data management and improve care coordination.

“By providing telehealth options, we have expanded access to care, particularly for patients who may face barriers to in-person visits.

“In addition, Third Street exchanges clinical information online which means that we collaborate with key providers in our healthcare network. This has been essential for ensuring the flow of care and improving patient outcomes.

“We have also been engaging patients through health information technology. We have made strides in involving our patients in their own care through various health tools such as MyChart, empowering them to take an active role in their health management.

“Finally, to earn the CHQR badge, we have been collecting data on patient social factors. Understanding the social determinants of health has allowed us to address the broader context of our patients’ well-being and tailor our services accordingly.

“For several years, we achieved the designation as a Patient Centered Medical Home (PCMH), which speaks to our commitment of putting patients at the center of their healthcare experience.

“This was a multidisciplinary effort and couldn’t have been achieved without everyone, however, we must acknowledge the efforts of our Clinical, Quality and Risk teams for their diligence and commitment to making this achievement possible.

“This recognition is a testament to our hard work and dedication to improving health services in our community. Third Street will continue to build on this success and strive for excellence in all that we do.”
